Running along 13400 North in the Cache County street grid, the highway also runs through the town of Lewiston along its 7.3-mile (11.75-kilometer) span, as well as Cornish and Richmond. Formed in the 1930s, the highway has not seen any major changes to its routing.

At the intersection of 4800 West (SR-23) and 13400 South in the center of Cornish, SR-61 departs east on 13400 South due east. Exiting Cornish, the highway crosses the Bear River and continues east through rural Cache County. The highway exits the town and ends at US-91 within 1 mile (1.61 km).
